The fastest way to apply for an Alert Card (including a replacement card) is by completing our simple online application form, either on your computer or via your smartphone/tablet. Please note our Alert Cards are available to UK residents only.

You will need the following before you start to apply for your FIRST CARD:

• A computer or smartphone/tablet with internet access (smartphones/tablets need a camera if taking a photo of diagnosis document)
• Proof of diagnosis - either electronic or in printed form
• A credit or debit card to pay for the card (£5 each)
• A valid email address
• Personal details of the person applying for the Alert Card

Apply for your replacement card

Please note that your alert card will not be posted to you until your application has been verified by us. Once applications have been processed, your card will be dispatched within 10 days.
